Insurance ID Card

How does it work?

Once you enroll in a health insurance plan and make your first payment, your insurance company sends you a membership card. This is proof that you have health insurance. This card will identify you to healthcare providers, who will use it for billing purposes and to confirm that they are part of your plan’s network. You should always take your membership card with you when you go to the doctor, hospital, lab, or pharmacy.

Step 1 - Find your Policy number

If you are our customer and don’t know your policy number, we can look it up for you. Simply, fill out this short form, and we will be in contact with you via phone or email.

Step 2 - Find Online to Access Your ID Card

Once you have your policy number you can create an online account at your insurance company’s portal and have access to your member ID Card.
